You can log in via ID-porten (electronic ID) for self-registration

As an ordinary part of the public health service, your health information is collected. The purpose is to obtain information in order to adapt correct health care.
As a parent, other relative or teacher, you may also be asked to fill out forms regarding your health information.

For privacy reasons, most of this is now done through a secure digital self-registration in CheckWare. As a patient, you will be asked to answer some information together with your doctor or therapist.

Log in here if you have received a one-time code from your doctor or therapist. This only applies to patients who do not have Electronic ID and only by agreements.

All other users must log in with Electronic ID via ID-porten.

About digital self-registration

Digital self-registration gives you access to forms that are necessary for you to fill in to get the best possible picture of what you, or the person you are filling in on behalf of, are struggling with. The form is completed on a secure website and the information is transferred directly to the patient record.

Who fills out the form?

As a patient, you will receive information from your place of treatment or directly from your doctor or therapist if you need to fill in the form using digital self-registration. If you have a role other than as a patient (for example as a parent, other relative or teacher), you may in some cases also be asked to fill in forms. If so, you will be informed of this.

How do I proceed?

You will receive an SMS with a link to login. Under "Login for patients or relatives", click on ID-Porten. There you log in with either BankID, BankID mobile, Buypass or Buypass mobile.

Children and teens who do not have electronic ID can, by agreement with the treatment centre, be given a one-time code. The link to ID-Porten can be found under "Login with one-time code".

You can login from PC, tablet or smartphone. After logging in, you will go directly to the form you need to answer. Your answers are stored on a secured server as you fill out the form. If you need to cancel, you can continue filling in later. When you have finished the answer, press "Done" and confirm that the survey will be submitted. If you need to answer multiple forms, these will open as the forms are delivered. It is not possible to change the answer to forms after you have submitted the form.

Important information for you as a patient

Once you have finished clicking and confirmed that the assessment will be submitted, it will be sent to your treatment provider. It may vary when the answers are reviewed, most likely at your next appointment.

Sometimes you may be asked to fill in the form via Digital self-registration before starting treatment. If so, you will be informed of this from your doctor or therapist. Login and procedure will be similar. Contact your doctor or therapist if you have been asked to use Digital Self-Registration before starting treatment and have questions about this.

You will be notified by your doctor or therapist if you are going to use Digital Self-Registration while you are being assessed or receiving treatment in the public health service. To ensure the quality of our treatment offer, you will sometimes be asked to complete the Digital Self-Registration for a period after the course of treatment has ended.
